Carolyn Tellez

My name is Carolyn Tellez, and I am a board certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). I am licensed in Arizona, Nevada, and Colorado. My focus is on medication management, but I always encourage incorporating lifestyle, and behavioral changes for the best outcome possible.

My approach

I work with a wide range of emotional and behavioral issues, providing a comprehensive evaluation. The client and provider will work together as a team to create the best treatment plan with reasonable and attainable goals.

Behavior Innovation specializes in online treatment for mental health disorders including, but not limited to: Depression, Anxiety, Panic Disorder, Bipolar Disorder, Schizophrenia, Insomnia, Attention Deficit Hyperactivity Disorder (ADHD), substance use disorders, and alcohol use disorder.

My communication style

Every person is an individual with unique needs, and there is no room for a cookie cutter approach when it comes to your mental health. Communicating with your provider should feel safe, nonjudgmental, and comfortable.

My journey to mental healthcare

I have been a nurse for many years, initially providing bedside care to the most critically ill in the Intensive Care Unit (ICU). I realized that my passion was in mental health and providing quality, evidence based, patient centered care to the people that may be having a tough time and feeling alone.

My goals for you

Goal setting should be unique to you, and what you hope to achieve. I can provide you with some guidance toward prioritizing, and together, we will plan, and implement. My main focus is to provide medication interventions to manage symptoms, and help you reach your goals.

My first session with you

Your first visit will be a comprehensive evaluation, including past history, as well as current symptoms that are causing difficulty. Medication options will be discussed, along with benefits, and risks. Goals will be set, and steps to get you there will be started.
